Case Study: Dave
Dave is a 33-year-old married man who was hit by a car a year ago while he was going to work. He was immediately taken to the hospital where he was initially diagnosed with a right occipital brain injury and bifrontal hematoma. He was unconscious for 5 weeks and the Glasgow Coma Scale indicated a score of 4. After Dave regained consciousness he was disorientated, and for the next 5 days, was unable to remember events, was unable to recognize his wife, was very confused, disinhibited, and anxious. He spent 6 more months at the hospital in-patient unit where he received rehabilitation.
Before the incident, Dave lived in Leicester in an apartment with his wife Olivia (32 years old). Currently, Dave has been discharged from the hospital 10 days ago, and he is back to his apartment. However, the rehabilitation team has serious concerns about Dave’s prognosis (which has been poor following the 6-month rehabilitation). Currently, Dave has severe difficulty in remembering things and also recalling events even within an hour of rehabilitation sessions. He is often unaware of which day it is and finds it difficult to read. He always needs one-directional cues to read. He has severe difficulties in his visuospatial functioning and he also struggles to perform everyday life activities like bathing and dressing.
Before the incident, Dave was working as a primary school teacher. His current difficulties in reading have severely affected his ability to return to work. His wife Olivia is also a primary school teacher and she is struggling to manage her work responsibilities along with the additional caring responsibilities. She manages to help Dave with everyday things like eating his meals, taking his medication, and helping him plan a routine for his day. However, she finds it very hard to help him with other more physically demanding chores like bathing and dressing. Olivia feels like she is now working two full-time jobs and has no time for herself. She feels more of a caregiver than Dave’s wife. She feels like Dave is not the same person anymore and that she has lost her husband in the incident. She often gets frustrated which leads to her having arguments with Dave.

Based on the brain injury details, give a detailed overview of the clients diagnosis. (approximately 800 words)
a) Provide a brief and clear history of the case.

2. Identify the appropriate neuropsychological assessment for the client and provide a brief rationale for it. (approximately 700 words)
a) Based on the current symptoms of the client, develop a detailed neuropsychological assessment battery.
b) Discuss what the specific assessments you have included and provide a rationale (relevant reasoning) of choosing them (why are the assessments important and how it will help the client).
c) Provide a brief and clear description/detail of the assessments.
Identify and critically evaluate the possible holistic rehabilitation strategies that may be useful to support the client. (1500 words)
a) Based on the current symptoms of the client, develop a rehabilitation (intervention) plan (specific intervention strategies for each symptom).
*Do not forget to develop a holistic rehabilitation (intervention) plan.
b) Provide a rationale for choosing each strategy.
c) Use clinical and research evidence to discuss in detail those strategies, their effectiveness, and critically evaluate them.
*This is an important part of the case study.
Conclusion: Even though this is not an important part of the case study, if possible, provide a short summary of the overall case and state the possible prognosis following the application of the intervention strategies.
