When you decide to purchase a new car, you first decide what is important to you. If mileage and dependability are the important factors, you will search for data focused more on these factors and less on color options and sound systems.

The same holds true when searching for research evidence to guide your clinical inquiry and professional decisions. Developing a formula for an answerable, researchable question that addresses your need will make the search process much more effective. One such formula is the PICO(T) format.

In this Discussion, you will transform a clinical inquiry into a searchable question in PICO(T) format, so you can search the electronic databases more effectively and efficiently. You will share this PICO(T) question and examine strategies you might use to increase the rigor and effectiveness of a database search on your PICO(T) question.

To Prepare:

• Review the Resources and identify a clinical issue of interest that can form the basis of a clinical inquiry.
• Review the materials offering guidance on using databases, performing keyword searches, and developing PICO(T) questions provided in the Resources.
• Based on the clinical issue of interest and using keywords related to the clinical issue of interest, search at least two different databases in the Walden Library to identify at least four relevant peer-reviewed articles related to your clinical issue of interest. You should not be using systematic reviews for this assignment, select original research articles.
• Review the Resources for guidance and develop a PICO(T) question of interest to you for further study. It is suggested that an Intervention-type PICOT question be developed as these seem to work best for this course.

Colleague 1

I have chosen the clinical topic of nursing handover. Every hospital appears to have their own way of how to perform a nursing handover. Change of shift is a crazy time as there are staff members from both the previous and oncoming shift. The craziness of shift change can impose the staff members from the shift leaving to give a report that lacks certain information.

This can be a lack of patient history background, lack of plan of care, at some facilities there is a lack of involving the patient in this transition of care. A lack in communication leads to errors in the hospital and therefore I found this topic to be intriguing. Ultimately leading me to ask, what is the best practice for nurses to handover at change of shift?

Is it at the bedside with the patient, half at the nurses’ station and half with the patient, bedside handover, or is there another way?  According to Stillwell et. al., (2010), a PICOT question involves a population, intervention or issue of interest, comparison intervention or issue of interest, outcomes of interest, and the time it takes to achieve those outcomes.

The question I proposed meets each of the PICOT qualifications as it includes the nurse as the population, the nursing handover as the issue of interest, best practice as the issue of interest and the outcomes, and change of shift as the time.

When beginning my search for articles regarding this topic, I started by going onto the Walden library homepage and searching by the nursing subject, through the Walden nursing database I began my search. I typed in the search bar “nursing handover at change of shift,” this populated 2,367 results.

This was without providing any parameters or using Boolean terms. According to the Library of Congress (n.d.), Boolean terms refers to using the words “AND” and “OR” to increase the strength of a search in databases. Once incorporating the words into my search, more results populated increasing to 9,454.

I decided that focusing on one database search engine such as CINAHL would also be a good place to look for research. I began my search the same way as I did on the other database engine, starting off with nursing handover at shift change. Without adding the parameters or the Boolean terms, there were 3, 195 results for the words “nursing report.”

Although those are a lot of results, they are not specific to my topic. I then added in the parameters and the Boolean term “OR,” the database populated 731 results. Although this is less results, this allows for more accurate results.

To increase the search results of my database searching more rigor and effective, I can use the Boolean terms, use relevant databases which focus on the clinical topic I have chosen, use the keywords from my PICOT question, and determine what evidence will best answer my question, Stillwell et. al., (2010).

Another resource to assist with finding acceptable evidence for my question would be asking the librarian at the Walden Library. When looking at how I could adjust my search terms, I could combine phrases from my question to get evidence that is more specific to my question, rather than include information that is not feasible to what I am looking for (Stillwell, et. al., 2010).

By combining these strategies, I will successfully find the information I am looking for to answer my clinical topic.

Clinical issue of interest: Does shaving the surgical site 24 hours before surgery decrease the risk of surgical site infections? Is it beneficial for patients to shave 24 hours before their surgery or immediately before their procedure?

P: population of interest: Patients scheduled for cesarean section.

I: intervention or issue of interest: Shaving 24 hours before surgery

C: comparison of interest: Shaving immediately before surgery.

O: outcome expected: Affect the rate of surgical site infections.

T: time for the intervention to achieve the outcome: 30 days after the surgical procedure.

Search terms used and database 1: Infection rate with shaving prior to cesarean section with EBSCOhost (CINAHL Plus with Full-Text database yielded 1146 results, but they were all varied and not tailored to my desired request.

Search terms used with Boolean operators, database 1: Shaving or removal hair or trim AND infection rates on EBSCO host (CINAHL Plus with Full Text) yielded 17 results, but they were specific and tailored to what I was searching.

Search terms used and database 2: Infection rate with shaving prior to cesarean section yielded nothing with Embase.

Search terms used with Boolean operators, database 2: Shaving or removal hair or trim AND infection rates on Embase database yielded 256 results.

Explain strategies to increase the rigor and effectiveness of a database search:

To increase the rigor and effectiveness of a database search, first determine your search terms by using your PICOT question as a guide. Secondly, consult a librarian or create the time to learn how to navigate different databases.
